The Prince of Wales Medical Centre has an opportunity for a full-time practice Nurse to join our team.
We are looking for an organised and enthusiastic person with at least 2 years post registration experience and a Post Registration GPN diploma or Practice Nursing Degree or equivalent. The successful candidate will be part of a well-supported clinical team, along with excellent admin and data support staff, serving 8500 + patients in our local area.
Main duties of the jobThe Practice Nurse will work with the Lead Practice Nurse andother members of the multi professional team, providing and monitoring the condition and health needs of patients within their care, contributing to the ongoing assessment, as agreed with supervising clinicians and in line with the service pathways. The primary clinics will be for management of long term conditions (Asthma, Diabetes and COPD), Child Health/Immunisation,Cervical Screening, Sexual Health screening and contraception but you will be expected oversee Wound Management and all other general treatment room duties.
Clinics will include health-checks for patients with Mental Health diagnoses and Learning Disabilities.
You will also assist the Lead Practice Nurse with CQC compliance and clinical stock control and ordering.

As a Practice Nurse you will act within your professional boundaries, providing care for patients from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of their careunder supervision
You will show safe clinical decision-making andexpert care for patients within the general practice, in consultation with theduty GP. You will work with the multi-disciplinary general practice team tosupport the delivery of policy and procedures and meet the needs of patients.
Supervision and mentoring will be provided by the appropriate senior medical staff - the type of support will depend on the postholders skills and knowledge and determined by our clinical governancearrangements.
Key Responsibilities

CLINICALRESPONSIBILITIES
Deliver high quality, compassionate care with a focus on promoting health, self-care, and independence.
Carry out specific delegated clinicaltasks and responsibilities to a competent standard
Working without direct supervision(where appropriate) within designated parameters of practice of the Practice Nurse role, accessing clinical and care advice when needed
Recognise issues relating to safeguarding vulnerable children and adults and report any problems or raiseconcerns to the appropriate registered care
Exercise professional accountability and comply with practice policy in relation to the safe administration of medicines
Use electronic records to record theoutcome of assessment and evaluation of care
Assist in the assessment of and management of risk across the environment where care is being administered
Assist with the implementation and monitoring of clinical standards and outcomes
Develop a working knowledge of other providers resources and referral systems to ensure individuals needs are met,within parameters of practice
Demonstrate good understanding ofprinciples of consent and ensure valid consent is obtained prior to undertakingnursing and care procedures
Demonstrate a good understanding ofthe Mental Capacity act / Deprivation of Liberties and applies principles toeveryday practice seeking advice / guidance from Registered Nurse or anotherregistered professional as required
