Overview
The Clinical Pathway Navigator plays a vital role in supporting the efficient management of patients through the Trauma & Orthopaedics service. Working closely with consultants, multidisciplinary teams, and administrative colleagues, the post holder will coordinate patient pathways from referral to treatment, ensuring that all stages of the process are completed in a timely and accurate manner.
Responsibilities- Monitor clinic outcomes, track investigations, book follow-up appointments, and liaise with patients and healthcare professionals to resolve queries and remove barriers to care.
- Coordinate patient pathways within the Trauma & Orthopaedics service from referral to treatment, ensuring all stages are completed within agreed timescales.
- Track and monitor outpatient appointments, diagnostic results, and follow-up actions, ensuring timely progression of patient care.
- Liaise with consultants, clinicians, nursing staff, physiotherapists, and administrative teams to resolve queries and remove delays.
- Maintain accurate records on the Trusts Electronic Patient Record (EPR) and other relevant systems, ensuring data quality standards are met.
- Proactively manage RTT (Referral to Treatment) targets and escalate potential breaches to the Service Manager or relevant lead in a timely manner.
- Communicate with patients in a clear and compassionate manner, providing updates on their care pathway, answering questions, and signposting as appropriate.
- Book and amend appointments in line with clinical priorities and Trust guidelines.
- Support virtual fracture clinics by preparing case notes, updating outcomes, and ensuring patients are contacted with their management plans.
- Work collaboratively with multidisciplinary teams to ensure smooth patient flow through clinics, theatres, and follow-up services.
- Contribute to service improvement initiatives by identifying process gaps and suggesting practical solutions.
- Adhere to confidentiality, data protection, and safeguarding policies at all times.
