About the Company
You will be joining a specialist, nurse-led community healthcare provider delivering complex, outcome-based care packages to adults and children in their own homes. The service works in close partnership with NHS continuing healthcare teams and is known for its commitment to high clinical standards, strong governance, and patient-centred care.
Key Responsibilities
Lead, manage, and develop Nurse Case Managers to ensure high-quality, consistent service delivery
Build trusted relationships with health and social care commissioners to support service growth
Oversee clinical governance, including incidents, complaints, and safeguarding processes
Maintain and develop clinical policies, procedures, and participation in a senior on-call rota About You
Registered General Nurse with current NMC registration
Senior-level experience in clinical or nursing management within community or complex care settings
Up-to-date hands-on experience with tracheostomy care and ventilator management
