Weare looking for an enthusiastic practice nurse to work in our friendly,spacious, purpose-designed, 4 partners General Practice located on Hampstead Heath.

To provide high quality nursing care to meet the needs of the practice population; which includes treatment, screening, preventative care, patient education, clinical quality systems maintenance; and the supervision of any less qualified clinicalcolleagues, while working in partnership with other clinical, management and administrative colleagues to meet stated practice developmental goals.

The duties will include all tasks normally undertaken by an experienced Practice Nurse and any additional rolesagreed between the nurse and the doctors as appropriate, having regard tocurrent training.

Main duties of the job

The successful candidate will ideally have several yearspost registration experience, be highly organised, motivated and computer literate. Previous experience of working in General Practice is essential,using EMIS Web would be desirable. Experience in childhood immunisation, womens health, woundcare, and general nursing essential. An interest in managing long term conditions such as diabetes is desirable.

You will be joining a nursing team consisting of two highly skilled Healthcare Assistants, and a Part Time Practice Nurse you will work as part of aprogressive, integrated and effective primary care team.

Job responsibilities

Professional:

To provide carefor minor injuries, dressings, including pressure bandaging and higher levelwound care, liaising directly, where appropriate with community and secondary care counter-parts, escalating to a GP as necessary and also provide supervision for less qualified nursing colleagues in the practice.

  • To undertake cervical cytology sampling,immunisations as outlined by national programmes and local guidelines,working closely with the nursing, clinical and admin team to createsystems to identify patients who are in need of such services and promoting the service hard to reach patients.
  • To provide support to GPs in the management of chronic diseases such as diabetes, COPD, Asthma, Hypertension etc. by providing specifically defined clinical sessions, working closely with theGPs to manage a patients overall care.
  • To support the annual flu campaign and othermass population vaccination programmes as needed in line with national guidance.
  • To act as a chaperone or similar support forother colleagues as needed.
  • To conduct and supervise other colleagues in anthropometric measurements, blood pressure, spirometry, urinalysis, phlebotomy, microbiology samples, ECG,ABPM, peak flow and other practice based tests as needed in the course ofthe care of patients.
  • To carry outhealth checks in conjunction with national, local and practice initiatives, setting up and maintaining a system for identifying patients in the target group for such checks.
  • To provide education to patients on practice and local health services, identifyingpatients at risk of adverse health behaviours such as physical inactivity,smoking alcohol and other substance use referring appropriately to othersif needed using protocols and standard that you help to establish.
  • Toeventually manage and maintain chronic disease registers and work withadmin staff to construct and maintain robust call and recall systems for chronic disease, cervical cytology, mammography, immunisation and otherpopulations based health promotion and disease prevention care programmes,escalating where necessary patients who need intervention from a GP toinsure a review as appropriate, constructing and maintain care pathwayswhere patients receive appropriate investigations and reviews by an appropriate clinician at the appropriate time interval.
  • To promote the health and wellbeing of patients by counselling patients on the natureof their conditions, the use of their medications, diet, exercise andother health behaviours.
  • Toeventually establish and participate in a robust system of clinical auditin partnership with other clinical and administrative colleagues, leadingin areas appropriate, such as infection control, to a nursing skill setand supporting in other areas such as medicines management.
