This is an exciting opportunity to join the CRSTAT (Community Rehabilitation short term assessment team).
We’re looking for a qualified, experienced Health and Adults Social Worker with substantial knowledge of adult social care legislation, principles and best practice. You will also need to be currently registered with Social Work England to join us. The ideal candidate will be committed to working creatively within the CRSTAT using the Strengths-Based and Person-Centred Approach to enable residents to experience personalised, integrated care.
This role also requires you to support residents in your professional practice to deliver a high-quality, robust, efficient and effective rehabilitation service that maximises individual user outcomes by enabling independence in collaboration with their family/carers and partnership organisations to support a strengths-based approach that minimises the need for long-term services.
This role supports residents within the CRSTAT remit to maintain independence and wellbeing through timely assessments of needs, implementation and review of packages of care, completing carers’ assessments, and working in collaboration with health and social care partners to cope with change, achieve outcomes, understand and manage risks related to social inclusion in the community, and facilitate and review care home placements where required.
You will have experience of assessing needs of adults and managing risks, including safeguarding enquiries. You’ll be responsible to the Team Manager and contribute towards the improvement and efficiency of the Team.
