Role Overview
The Senior Nurse Practitioner will work in partnership with service users and carers in the Older People Community Mental Health Service. The post involves leading the team, managing the day‑to‑day running of the service, and ensuring patients have personalised care plans to enable them to lead fulfilling lives.
Key Responsibilities- Lead and supervise the nursing team, including clinical supervision of staff and students.
- Assess, plan and implement comprehensive care for older people with complex mental health needs, including dementia. Provide specialist nursing advice, discharge planning and manage a caseload of clients.
- Provide clinical supervision of staff and students and chair meetings, covering the Team Manager as required.
- Develop and implement practice development activities and training for other professionals, students and agencies.
- Administer medication within NMC guidelines and the Trust’s Medicines Code, and work in partnership with other agencies to achieve person‑centred outcomes.
- Promote the safety and well‑being of service users and their families/carers.
- Experience of and specialist knowledge in interventions for older people with complex mental health needs and dementia.
- Knowledge of both organic and functional illnesses.
- Demonstrated excellent communication skills and the ability to communicate with and meet the needs of families and carers.
- Experience of managing and supervising other members of the team.
- Experience of leading a shared duty service and undertaking case management/lead professional/ CPA as needed.
- Strong organisational and professional link‑building skills.
