Role Overview
Agency Social Worker – Hospital Discharge / At Home Service 📍 SE27, London 💷 £30–£32 per hour (DOE) 🕒 Full-time | 35 hours per week | Monday–Friday 📅

Contract:
Agency (initially until end of March 2026)
- Care Act assessments and reviews
- Risk assessments and safeguarding work
- Discharge planning and admission avoidance
- Short‑term interventions to support people at home
- Close partnership working with health colleagues and wider professionals
Office‑based 5 days per week during triage/duty weeks (approximately one week per month). Hybrid working during non‑duty weeks, with a minimum of 3 office‑based days per week.
Qualifications & Requirements- Social Work England registration
- Enhanced DBS
- Proven experience in hospital discharge, intermediate care, reablement, or front‑door services
- Strong working knowledge of the Care Act, safeguarding, and proportionate risk management
- Ability to manage complex, high‑volume caseloads in a time‑pressured environment
- Excellent communication skills and commitment to Equality, Diversity, and Inclusion
Funding for the role is currently confirmed until 31 March 2026, with an extension anticipated and awaiting confirmation.
