Ealing Short Term Rehabilitation – Band 6 Community Occupational Therapist
Are you an innovative, organised and motivated occupational therapist with a strong interest in community rehabilitation? We have exciting opportunities for a fixed‑term Band 6 occupational therapist to join our specialist multidisciplinary community rehabilitation service.
Please note that our service has two bases and interviews will be held in person at our office in Level 10 South Ealing Hospital, London or at the EDTC, Southall UB1 2SH

The Ealing Short Term Rehabilitation (STR) team provides therapy‑led rehabilitation to help patients improve their abilities and achieve their rehabilitation goals, through therapy‑led care. We focus on achieving goals and enabling people to regain independence and social integration effectively and efficiently after discharge from acute hospitals and community services in the borough. Therapists and rehabilitation assistants offer support for a period of up to six weeks, usually less if goals are achieved or care can be delivered in a different way.
Our team includes Physiotherapists, Occupational Therapists, Rehabilitation Assistants. The service is delivered in a variety of community locations, mainly in patients’ own homes.
- Provide specialised rehabilitation assessment, education, advice and treatment to adult patients in the community who have complex health and social care needs.
- Prioritise factors involved in the patient’s presenting condition through clinical reasoning based on knowledge of current professional opinion, prognosis and other physical, social, emotional and mental health issues.
- Set goals for treatment with patients and/or their carer and ensure that care is patient‑centred and agreed by the patient and/or their carer.
- Independently screen and assess multi‑disciplinary referrals from a variety of sources, establishing priority, determining needs and initiating a plan of intervention.
- Manage a caseload of patients with a range of needs, utilising supervision and guidance as necessary from senior staff.
- Work in a variety of community locations including patients’ homes, nursing homes, and residential homes and provide intensive rehabilitation.
- Successfully monitor, evaluate, and review occupational therapy treatment/interventions within a multi‑disciplinary framework and modify as necessary, taking a key worker role as indicated.
- Ensure patient and service records are recorded and maintained on IT databases and that patient documentation is in accordance with trust and local policies.
- Prescribe, delegate and monitor the work of rehabilitation assistants and counter‑sign documentation.
- Actively be involved in service development, with support and guidance from senior therapy staff.
- Participate in departmental continuous professional development, education and training, audit and research projects.
- Maintain and update a Continuing Professional Development Portfolio (CPD).
- Deliver skilled OT care to patients in their own homes based on a thorough holistic assessment of their clinical and personal needs and an individualised rehabilitation care plan, meeting the standards set by RCOT and HCPC.
- Participate in service‑wide seven‑day working on a rota basis.
