Summary

• This 2-3 month contract with a local authority aims to support care leavers in their transition to adulthood. The role is crucial in providing high-quality services working collaboratively with families statutory agencies and third-sector organizations to ensure positive outcomes for care leavers.

• Deliver high-quality services to care leavers ensuring successful transitions into adulthood.
• Prepare review and implement Pathway Plans in collaboration with the supervising manager ensuring parallel planning where applicable.
• Conduct human rights assessments to uphold and promote the rights of young people.
• Coordinate service provision and ensure care leavers access available services.
• Maintain accurate records of all contacts interventions and services on the care leavers electronic case file.
• Work holistically with care leavers and their families during transitions or transfers to additional servicesli>

• Minimum of 2 years experience working with young people in a supportive role within a voluntary or statutory setting.
• Proven ability to empower and support a person-centred approach in developing planning and reviewing support plans.

• Level 3 qualification or above in Social Care Youth Work Psychology Education or a related field (or equivalent experience).

• Working hours:
  
  36 hours per week
• The role requires a DBS check.
