Position: Op RESTORE Pathway Navigator | Imperial College Healthcare NHS Trust

Overview

Op RESTORE is an NHS service hosted by Imperial College Healthcare NHS Trust that supports individuals who have served in, or are leaving, the UK Armed Forces and have continuing physical health injuries and related medical problems attributed to their time in the Armed Forces.

This is a homeworking post based at the post holder's home with occasional travel into London or other parts of England. It is a non-clinical role with a strong admin focus, requiring comfort working independently. The successful candidate will have experience in healthcare navigation to support the roll-out of Veteran-Initiated Referrals and will liaise with veterans on the phone as well as GP practices, military charities and NHS Trusts.

Responsibilities
  • Develop, support and implement plans for providing first contact with veterans referred into the service.
  • Assist with completing assessment checklists and support the onward navigation of statutory and non-statutory services in support of the service.
  • Support referrals into the service and contribute to the overview and direction of onward clinical pathways for veterans.
  • Liaise with veterans, GP practices, military charities and NHS Trusts to facilitate effective service delivery.
  • Contribute to the roll-out of Veteran-Initiated Referrals over the coming year.
Requirements / Qualifications
  • Strong administrative background and comfort working on your own.
  • Experience in healthcare navigation or related client-facing roles.
  • Interest or passion for veteran healthcare or experience of working in the Armed Forces.
