We are looking to recruit a Qualified Social Worker in the Hospital Discharge Team within Hackney. The successful candidate will be working in a well-structured, organised team who are growing their social worker staffing levels. Responsibilities- Assess the social care needs of adults in an acute hospital setting to support timely, safe discharge
- Complete Care Act assessments, mental capacity assessments, and best interest decisions in line with legislation
- Work collaboratively with health professionals, families, and partner agencies to coordinate discharge planning
- Arrange appropriate packages of care, placements, and community support services
- Safeguard adults at risk and respond to concerns in line with statutory duties and local procedures
- Contribute to multidisciplinary team meetings and discharge planning forums
- Maintain accurate, timely, and legally compliant case records
- Promote strengths-based, person-centred practice and support service user choice and independence
- Manage a complex caseload, prioritising effectively to meet service demands and discharge targets.
- Social Work England registration
- Eligibility to work in the UK
- Social Work qualified (MA/BSc/DipSW/CQSW/MSc/BA)
