Overview

An exciting opportunity has arisen to join the Adult Community Nursing Service. We are seeking enthusiastic Band 5 nurses to fill full-time (37.5 hours) posts. The Adult Community Nursing Service is divided into 8 neighbourhood teams; the successful candidate will work within one of these teams.
Jobresponsibilities
- The post holder will practice within a multidisciplinary team and provide care to patients within their own home and community settings.
- Assessing, planning, implementing and evaluating care needs under the supervision of Clinical Nurse Specialists, Practitioners and District Nurses.
- Deliver a high standard of holistic nursing care while promoting health education and independence.
- Take responsibility for own actions when delivering clinical care; maintain competency and skills in line with role requirements.
