Thames Valley Family Health Team (TVFHT) is one of the largest and most comprehensive family health teams in Ontario, serving patients across more than 15 sites in four counties. Our mission is to provide collaborative, patient-centered care through an interprofessional team approach that emphasizes innovation, access, equity, and excellence.
Family Health Team staff includes nurse practitioners, registered nurses, social workers, pharmacists, dietitians, occupational therapists and other health care professionals who will work with physicians as an interdisciplinary team, each using their experience and skills to provide patient centred care. RequirementsAcademic preparation from an accredited Canadian university program in foods & nutrition or equivalent
Practical training in an accredited practical program in Canada or equivalent
Three to five years clinical counseling and community nutrition experience or comparable skills, experience and education
Experience in program development, implementation, monitoring and evaluation
Member in good standing with College of Dietitians
Valid driver’s license, regular access to a vehicle, and appropriate insurance.
- Ability to work in clinic independently in a team-based, fast-paced environment
Knowledge and proficiency in current, evidenced-based methods and practices of primary care delivery, with an emphasis on health promotion and risk reduction
Strong leadership, organization, evaluation, time management, communication and interpersonal skills
Experience in the use of EMR and Microsoft Office applications
Desire and ability to update knowledge and skills through various means including technology-based opportunities, courses, workshops and conferences
Multi-tasking skills related to the delivery of efficient primary care
Excellent verbal and written communication skills
Exhibit an ability to be open and non-judgmental
Knowledge of the goals and structure of the organization
Solid understanding and experience with adult group learning principles, teaching self management techniques & ability to evoke participation in a group environment as group facilitation will be part of this role
Level of comfort or willingness to become familiar with virtual meeting technology
Experience with facilitation of an online program considered an asset
Experience and commitment to working collaboratively with other healthcare professionals
