Overview
Belgrave Medical Centre is seeking a motivated and patient-focused Primary Care Clinical Pharmacist to join our established multidisciplinary team. This role offers an excellent opportunity to contribute to high-quality, safe, and effective medicines optimisation within a busy general practice setting.
The post holder will work closely with GPs, nurses, allied health professionals, and the wider primary care team to support medication management, deliver structured long-term condition reviews, and contribute to clinical audits and service improvement. The role is integral to improving patient outcomes, reducing medicines-related risk, and supporting proactive, personalised care across the practice population.
Main duties of the job- Undertake structured medication reviews, including for patients with long-term conditions, polypharmacy, frailty, and high-risk medicines, in line with national and local guidance.
- Support the management of long-term conditions such as hypertension, diabetes, asthma, COPD, and cardiovascular disease, ensuring safe, effective, and evidence-based prescribing.
- Provide clinical advice and support to GPs, nursing staff, and other members of the practice team on medicines management and prescribing queries.
- Implement and support medicines optimisation initiatives in line with NICE guidance, local formularies, and ICB policies.
- Contribute to the delivery of clinical audits, quality improvement projects, and prescribing reviews to support safe and effective practice.
- Support prescribing safety activities, including high-risk drug monitoring, medicines safety alerts, and recall processes.
- Liaise with community pharmacies, secondary care, and other external services to ensure continuity of care following hospital discharge and outpatient appointments.
- Support repeat prescribing processes, ensuring timely, safe, and appropriate authorisation in line with practice protocols.
- Maintain accurate and up-to-date clinical records in accordance with professional standards and information governance requirements.
- Participate in clinical meetings, training sessions, and multidisciplinary discussions as required.

Professional Responsibilities

- Maintain registration with the General Pharmaceutical Council (GPhC) and adhere to professional standards of practice.
- Work within the scope of competence and escalate clinical concerns appropriately.

- Qualifications & Registration:
Registered Pharmacist with the General Pharmaceutical Council (GPhC); MPharm degree or equivalent recognised pharmacy qualification;
Evidence of ongoing Continuing Professional Development (CPD). - Experience:
Experience of working as a pharmacist in a patient-facing role;
Experience of medicines optimisation and medication review in primary care, community, or hospital settings;
Experience of managing patients with long-term conditions and polypharmacy;
Experience of working collaboratively within a multidisciplinary team. - Knowledge &
Skills:
Sound knowledge of clinical therapeutics and evidence-based prescribing;
Understanding of medicines management in primary care, including repeat prescribing processes;
Ability to identify and manage medicines-related risks, including high-risk medicines and adverse drug reactions;
Ability to undertake structured medication reviews and support long-term condition management;
Strong clinical assessment, problem-solving, and decision-making skills;
Good written and verbal communication skills, with the ability to communicate effectively with patients and healthcare professionals. - Personal Attributes:
