Overview

An exciting opportunity to join a wonderful, dynamic and patient-focused team providing high quality services to the local community. As a Community Nurse you will deliver a high standard of care to patients in their homes while developing your leadership skills supporting your team. This role requires travel around the local community and participation in a multi‑professional environment.

Responsibilities
  • Lead a community nursing team within a multi-professional environment.
  • Provide care to patients in their homes, including supporting team members with new skills and advising families as needed.
  • Deliver a range of clinical duties including wound care, catheter care, palliative care, IV drug administration, medication/injection administration, and diabetes management.
  • Support weekend, late shifts and bank holidays on a pro rata basis; late shift until 8pm.
  • Collaborate with Local Authorities, Integrated Care Networks, GPs, and Acute sector providers to facilitate care pathways that support early discharge and prevent hospital admission.
  • Newly qualified staff will have access to a preceptorship program and a specific clinical upskilling training and PNDs support. A two-week Trust induction and two weeks local team induction will be provided.
Requirements
  • Registered Nurse (RN).
  • Full driving licence and access to a car; travel around the local community is required.
  • The service covers 8am–8pm, seven days per week; shifts are 8am–4pm and 12pm–8pm, full-time including weekends.
  • Evidence-based care experience in wound care, catheter care, palliative care, IV drug administration, pharmacological administration, diabetes management, and related skills.
Team/Clinical Environment

As a member of our community multi-disciplinary team you will work within an integrated team of District Nurses, Specialist Nurses, Community Matrons, Therapists, and Health Care Assistants to support high-quality evidence-based care for the local community.

You will work in collaboration with Local Authorities, Integrated Care Networks, GP practices, and Acute sector providers to facilitate care pathways that support early discharge and prevent hospital admission.
