Job Description And Main Responsibilities
Provide a range of evidence‑based psycho‑social interventions and proactive risk‑management approaches to help service users and their families reduce and manage symptoms, high‑risk behaviour, and the disabling effects of psychosis. Work collaboratively with the Operational Team Manager, Clinical Leads/Senior Practitioners, Inpatient Service and Home Treatment Team. Support clients in relapse prevention and crisis planning, coordinating interventions to maximise home‑care and avoid hospitalisation.
Additional responsibilities include:
- Ensure safe and effective care in line with CNWL Child Protection Guidelines and safeguarding procedures.
- Deliver ongoing psycho‑education and health promotion to service users and families.
- Work with multidisciplinary teams on co‑morbid problems including substance use/misuse.
- Act as lead or member of triage team to manage caseloads and provide close working relationships with inpatient and home treatment services.
Essential criteria
- Current professional qualification in mental health nursing; registration must be live with the NMC.
- Evidence of post‑registration training and Continuing Professional Development (CPD).
Desirable criteria
- Training in psychosocial interventions, Thorne Family Therapy/Work training, or other relevant substance misuse training.
Essential criteria
- Significant post‑registration experience working in a multidisciplinary mental health team with clients experiencing acute, severe and enduring mental health problems.
Desirable criteria
- Experience with homeless or rootless clients, crisis work, supervision of students and junior staff, audit, research, teaching, or substance‑use work.
Essential criteria
- Excellent verbal and written communication skills, including report writing and presentations.
- Strong IT skills.
- Ability to assess, plan, implement and evaluate a comprehensive care package for identified client groups.
- Capability to develop, implement, and review intervention programmes for client groups and their families.
- Demonstrated understanding of the needs of diverse populations, including inner‑city residents, BAME groups.
- Proficiency in building constructive relationships with clients and carers using warmth, empathy and effective communication.
Desirable criteria
- Leadership skills – motivation, initiative, and change management.
- Dual diagnosis/substance misuse expertise.
- Basic counselling and group work skills.
