Overview

Candidates are invited to apply for a Fellowship in advanced luminal endoscopy. The successful candidate will gain exposure to and receive training in a number of advanced luminal endoscopy techniques including UGI EMR and Barrett’s radio‑frequency ablation, colonic EMR and advanced polypectomy, balloon enteroscopy and capsule endoscopy. They will also gain experience in unit management and governance and will have the opportunity to participate in research.

It is hoped the candidate will also develop their teaching skills during the placement. The post will have 2 dedicated training lists per week and will include a service commitment of 5‑6 lists per week of mixed endoscopy with flexible working to cover consultant leave and ward commitments. The post also includes responsibility for the organisation and running of the complex polyp MDT.

Applicants should be at or near the end of their specialist training and independent in these activities. The candidate should be JAG signed off for standard therapeutics with audit data satisfying standard JAG auditable outcomes. Candidates will also take part in the on‑call GI bleeding rota with associated banding.

Key Responsibilities
  • The post holder will work closely with other members of the team to ensure high‑quality luminal endoscopy and gastroenterology services within the strategic framework agreed by the team and will work closely with the appropriate clinical management teams.
  • Perform 5‑6 luminal endoscopy sessions per week, mostly back‑filling available slots in the endoscopy schedule.
  • Attend 2 teaching lists per week in order to obtain experience in complex procedures such as UGI EUS, Barrett’s RFA, colorectal and UGI EMR and balloon enteroscopy.
  • Attend the UGI and LGI MDT.
  • Contribute to service development initiatives.
  • Contribute to service governance and the clinical management of endoscopy.
  • Participate in the teaching and training of undergraduate and junior medical staff.
  • Participate in ongoing research within the department.
  • Commitment to the out‑of‑hours endoscopy service at either SpR or Consultant level depending on the seniority of the post holder.
  • Support the junior clinical team and nurses with decision‑making.
  • Achieve clinical efficiency (e.g. length of stay reductions, reduction of cancelled procedures and DNA rates).
  • Achieve quality outcomes (e.g. infection control targets, reduction of re‑admission rates).
  • Manage financial aspects (e.g. identification, implementation and achievement of cost improvement programmes; participation in efforts to ensure services are provided cost‑effectively, managing locum agency spend, monitoring and managing the drug budget to target, ensuring accuracy of clinical data for the team).
  • Improve operational efficiency (e.g. day case rates, waiting list activity and demand management).
  • Participate in clinical audit, incident reporting and analysis and ensure resulting actions are implemented.
  • Work closely with the Directorate, Patient and Public Involvement panels in relation to clinical and service developments as delegated by the Divisional management team.
  • Ensure NICE clinical requirements are reviewed, implemented and monitored.
  • Ensure clinical guidelines and protocols are adhered to by junior medical staff and updated on a regular basis.
Qualifications and Requirements

Candidates must have full MRCP or equivalent. Applicants must have demonstrable skills in listening, reading, writing and speaking in English that enable effective communication about medical topics with patients and colleagues, as set out in the GMC’s Good Medical Practice (2013). The candidate should be JAG signed off for standard therapeutics with audit data satisfying standard JAG auditable outcomes and be at or near the end of specialist training and independent in these activities.
