The Thames Valley Family Health Team is one of the largest family health teams in Ontario, with more than 125 physicians across 25 sites within London, and Middlesex, Oxford, and Elgin counties.

We are seeking a dedicated and compassionate Registered Nurse (RN) or Registered Practical Nurse (RPN) to deliver foot care and wound care services. This position offers a unique opportunity to make a difference in the lives of those impacted by mental illness, substance use, poverty, and complex and/or chronic health conditions while being supported by a team of clinicians, Outreach Workers, and community partners.

Skills & Qualifications Required
  • Current registration and in good standing with the College of Nurses of Ontario
  • National Competencies for Advanced Nursing Foot Care in Canada (CAFCN)
  • Demonstrated experience in Community and/or Primary Care setting
  • Experience in wound care assessment and treatment planning
  • Experience working with Indigenous communities and vulnerable marginalized populations
  • Fluency in local indigenous language an asset
  • Best Practice Guidelines in Wound Care (RNAO & provincial standards)
  • Specialized training in providing comprehensive foot care is required
  • Knowledge and proficiency in current, evidence‑based methods and practices of primary care delivery, with an emphasis on health promotion and risk reduction
  • Strong leadership, organizational, research, evaluation, time management, and interpersonal skills
  • Experience using EMRs and Microsoft Word, Excel, and Outlook
  • Desire and ability to update knowledge and skills through technology‑based opportunities, courses, workshops, and conferences
  • Multi‑tasking skills related to the delivery of efficient primary care, including the ability to share information and teach while treating a patient
  • Ability to build trusting relationships with patients facing systemic barriers, trauma, or intergenerational impacts, through the provision of trauma‑informed, patient‑centred care
  • Strong assessment and clinical judgment in wound care, pressure ulcer prevention, and diabetes‑related foot complications
  • Excellent verbal and written communication skills
  • Exhibit an ability to be open and non‑judgmental
  • Experience and commitment to working collaboratively with other healthcare professionals
  • Experience working with a diverse population of patients with chronic disease, vascular disease, and those at high risk for foot complications
  • Competence and care in the treatment of foot complications
  • Healthcare system navigation and knowledge of community resources
  • Must have access to reliable transportation as travel between sites is required
  • This position is 100% on site
Compensation

RPN Hourly Rate: $28.37‑$34.60 (rate dependent on experience)

RN Hourly Rate: $34.81‑$42.46 (rate dependent on experience)
