Patient Care Assistant (1.0 FTE, ongoing contract)The Thames Valley Family Health Team is one of the largest family health teams in Ontario. With more than 125 physicians across 21 sites within London, and Middlesex, Oxford and Elgin counties, the Thames Valley Family Health Team is committed to providing comprehensive inter-collaborative primary health care to over 162,000 patients.
Responsibilities- Provide patient care and support as part of the Integrated Primary Healthcare Team
- Assist with administrative tasks and facilitate seamless communication within the team
- Support on-site operations alongside Nurse Practitioners and the interdisciplinary healthcare team
- Report to the team and align with TVFHT Health and Safety policies and procedures
- Be available for some evening hours and schedule vacation in alignment with the NP team schedule
- Work on-site with TVFHT’s Operations Department to offer administrative and clinical patient support in a primary care setting
- Administrative experience in a healthcare setting
- Post-Secondary school diploma from a medical assistant program;
Certified Medical Assistant (CMA) or equivalent preferred - Experience providing injections or certificate of completion to administer injections (preferred)
- Excellent verbal and written communication skills
- Excellent interpersonal skills and experience working in a multi-disciplinary team
- Strong attention to detail and ability to prioritize in a fast-paced environment
- Understanding of medical terminology and clinical procedures
- Proficiency in using electronic medical records (EMR) systems and computer skills
- Experience with Outlook, Teams and Microsoft applications (Excel, Word, PowerPoint)
- Understanding of the Ministry of Health and Family Health Team philosophy
- Ability to work independently with minimal supervision
- Ability to coordinate and communicate information and documentation for meetings and events, including minute taking
- Proficiency in data mining and compiling information for inclusion in reports
- Excellent critical thinking skills to anticipate and respond to administrative and clinical needs
- Ability to protect confidential and sensitive information related to patients and personnel with understanding of PHIPA
- Availability to open and/or close the clinic with some evening hours required
- Ability to monitor, track and follow up with patients on referrals initiated by NP(s)
- Reliable transportation to travel to other locations as needed
Salary Range: $47,046 - $57,374
