RN Case Manager, Hospice Per-Diem

We are hiring RN Case Managers to join our Hospice Team in Lompoc. We have Per‑Diem and Full‑Time opportunities available.
There might also be an opportunity to pick up Home Health visits too!
$81 per/routine and $141 per/eval visits.

• Have flexible hours and working close to home (reduce your commute!)
• Complete initial and on‑going comprehensive assessments of the patient’s needs
• Provide service and /or treatments requiring substantial and specialized nursing skills
• Initiate appropriate preventative and rehabilitative nursing procedures
• Counsel, support, and instruct the patient, family and caregiver in meeting nursing and related needs

• Graduate of an approved school of professional nursing.
• Current, active license in the state of California Board of Nursing,
• Current CPR, health clearance, TB clearance.
