Home Health Care Registered Nurse - QA RN
Location: Lombard, IL (On-Site)

About UsSafe Life Home Health, a reputable home health company with a 4-star patient satisfaction rating, is excited to announce an immediate opening for an experienced QA Nurse to join our dynamic clinical team. We are looking for a dedicated professional who shares our commitment to providing exceptional home health care.
Responsibilities- Oversee the integrity of the chart including accuracy of OASIS, regular visit notes, supervisory visits, Eval/Re-eval, discharge visit note, transfer / discharge summary, Physician orders / Plan of Care, communication notes, infection/fall logs, NOMNC form, etc.
- Chart audit review and addressing areas of concern.
- Knowledgeable of OASIS (SOC/ROC/Recert/DC).
- PCR and ADR preparation and submission.
- Clinical Record Review (CRR).
- Resource for resolving issues related to patient documentation.
- Knowledge of various plan reimbursements (Medicare, Medicare Advantage, Medicaid, Private PPO/HMO Plans).
- Review and evaluate all disciplines for quality, cost-efficient care plans.
- Participate weekly / monthly meetings and assist with quality assurance and improvement efforts.
- Knowledge of IDPH, CHAPS, CMS Surveys/Audits is a plus.
- Familiar with Financial Reimbursement Systems, PDGM, Case mix.
- Active RN license in the state of Illinois.
- Proven expertise in QA for Home Health and OASIS-E review.
- Proven experience with Kinnser EMR Software (Wellsky).
- Strong organizational skills and detail-oriented with a solid understanding of home health care.
- Knowledgeable about Medicare home health requirements.
- Positive, professional demeanor, excellent team-building, communication, and interpersonal skills.
- Highly and consistently organized.
- Coding and ICD-10 knowledge is advantageous.
- Excellent communication and time-management skills.
