Safe Life Home Health Care, LLC was founded in 2010. We are professional, customer-centric, fast-paced home health agency dedicated to providing state-licensed, CHAP-accredited, and Medicare-certified care. Our team includes clinicians with diverse skills across wound care, lymphedema management, and other therapy specialties, supported by a coordinated, multidisciplinary approach to patient care. We currently serve 13 counties in Illinois:
Cook, DuPage, Kane, Kendall, Will, McHenry, Lake, DeKalb, Boone, Grundy, LaSalle, Lee & Ogle.
Provides Occupational Therapy services to patients as directed by Plan of Care in line with the philosophy, objectives, and policies of the agency. Supervises Occupational Therapy Assistants and maintains responsibility for the implementation of follow-through of OT procedures.
Responsibilities- Assists the physician in evaluating patients’ level of function, establishing treatment plan and goals.
- Performs initial evaluations within 48 hours of referral and re-evaluations every 30 days, coordinating plan of care with the physician as needed.
- Prepares clinical and progress notes and required summaries, as appropriate.
- Revises plan of treatment and contacts the physician for changes in plan of care.
- Advises and consults with patient’s family and other health care staff.
- Coordinates care with the agency DON and other staff as appropriate.
- Observes, records, and reports changes in the patient’s condition and updates the agency on changes in status or schedule.
- Supervises the Occupational Therapy Assistant.
- Instructs patient and/or family in the use of equipment, safe patient management, and home therapy programs as needed.
- Communicates effectively with patients, care givers, families, and agency staff.
- Provides quality and appropriate care and reports issues or concerns to the DON/Administrator.
- Ensures all tasks are performed in a timely manner.
- Checks in on Occupational Therapy Assistants every 4-6 patient visits.
- Must be a licensed Occupational Therapist by the State of Illinois.
- Must have at least one-year Home Health Care clinical experience.
- Car with required proof of insurance coverage.
- Maintains annual TB test, CPR certification and other requirements.
- Able to read and interpret instructions related to patient care.
- Able to effectively perform and prioritize multiple functions or tasks as needed.
- Must be able to travel to prospective patients’ place of residence.
