Currently covering 13 counties in Illinois:
Cook, DuPage, Kane, Kendall, Will, McHenry, Lake, DeKalb, Boone, Grundy, LaSalle, Lee & Ogle.
Provides Physical Therapy services to patients under the direction and supervision of a licensed Physical Therapist, in accordance with the plan of care and in line with the philosophy, objectives, and policies of the agency.
Responsibilities- Implements the treatment plan established by the supervising Physical Therapist.
- Provides physical therapy treatments and procedures as outlined in the plan of care.
- Assists patients with therapeutic exercises, gait training, transfer training, and other interventions as directed.
- Documents patient progress, treatment provided, and any changes in patient condition.
- Reports observations and patient responses to the supervising Physical Therapist.
- Communicates effectively with patients, caregivers, families, and agency staff.
- Instructs patients and/or family members in home exercise programs and safe use of equipment as directed by the PT.
- Coordinates care with agency DON and other staff as appropriate.
- Observes, records, and reports any changes in patient condition to the supervising PT and agency.
- Participates in supervisory visits with the Physical Therapist every 4-6 patient visits.
- Assures that all tasks are performed in a timely manner.
- Provides quality and appropriate care and reports issues or concerns to the DON/Administrator.
- Must be a licensed Physical Therapist Assistant by the State of Illinois.
- Must have at least one year of Home Health Care clinical experience.
- Car with required proof of insurance coverage.
- Maintains annual TB test, CPR certification, and other requirements.
- Able to read and interpret instructions related to patient care.
- Able to effectively perform and prioritize multiple functions or tasks as needed.
- Must be able to travel to patient's place of residence.

Pay: $65,000 - $75,000 per year
Schedule: Monday to Friday;
Weekends as needed

Experience: Minimum 1 year Home Health experience required
License/Certification: Licensed PTA in State of Illinois
