Overview

Job Title: Home Health Aide / CNA

Location: Des Plaines, IL and surrounding areas

Type: Full-time/Part-time

• Assist with Activities of Daily Living (ADLs): bathing, dressing, grooming, and toileting for home health patients.
• Mobility Support: help patients with walking, transferring, and using mobility aids.
• Vital Sign Monitoring: take and record vital signs, including blood pressure, pulse, and temperature.
• Medication Reminders: assist patients with medication reminders and ensure compliance with medication schedules.
• Nutrition Assistance: help with meal preparation, feeding, and monitoring dietary restrictions as prescribed.
• Hygiene and Comfort: maintain patient hygiene and comfort, including changing bed linens and providing oral care.
• Observation and Reporting: observe and report changes in the patient's condition, behavior, or symptoms to the nursing team.
• Documentation: accurately document care provided, vital signs, and any changes in the patient s condition in the EMR system (Kinnser by Well Sky).
• Communication: maintain clear and effective communication with the patient, their family, and the healthcare team.
• Safety Measures: ensure a safe environment for the patient by following infection control and safety protocols.
• Transportation: provide transportation to medical appointments or assist with mobility as needed.
• Emotional Support: offer emotional support and companionship to patients, promoting their overall well-being.
• Collaboration:
  
  collaborate with the healthcare team, including nurses and therapists, to provide comprehensive care to patients.
• Patient
  
  Education:
  
  educate patients and their families on self-care and safety measures.
• Compassion: show compassion, kindness, and empathy while providing care to home health patients, who may be dealing with pain or illness.

• Active CNA certificate
• Valid driver s license and car insurance
• CPR certification
• Prior experience in home health is a plus
• Familiarity with EMR Kinnser by Well Sky is advantageous
• Excellent communication skills
