Currently covering 13 counties in Illinois:
Cook, DuPage, Kane, Kendall, Will, McHenry, Lake, DeKalb, Boone, Grundy, LaSalle, Lee & Ogle.
Provides Speech-Language Pathology services to patients under the direction and supervision of a licensed Speech-Language Pathologist, in accordance with the plan of care and in line with the philosophy, objectives, and policies of the agency.
ResponsibilitiesImplements the treatment plan established by the supervising Speech-Language Pathologist.
Provides speech therapy treatments and interventions as outlined in the plan of care.
Assists patients with speech, language, cognitive-communication, and swallowing exercises as directed.
Conducts therapy activities focused on articulation, fluency, voice, and language skills.
Documents patient progress, treatment provided, and any changes in patient condition.
Reports observations and patient responses to the supervising Speech-Language Pathologist.
Communicates effectively with patients, caregivers, families, and agency staff.
Instructs patients and/or family members in home exercise programs as directed by the SLP.
Coordinates care with agency DON and other staff as appropriate.
Observes, records, and reports any changes in patient condition to the supervising SLP and agency.
Participates in supervisory visits with the Speech-Language Pathologist as required.
Assures that all tasks are performed in a timely manner.
Provides quality and appropriate care and reports issues or concerns to the DON/Administrator.
Must be a licensed Speech-Language Pathology Assistant (SLPA) by the State of Illinois.
Must have at least one year of Home Health Care clinical experience.
Car with required proof of insurance coverage.
Maintains annual TB test, CPR certification, and other requirements.
Able to read and interpret instructions related to patient care.
Able to effectively perform and prioritize multiple functions or tasks as needed.
Must be able to travel to patient's place of residence.

Pay: $65,000 - $75,000 per year
Schedule: Monday to Friday;
Weekends as needed

Experience: Minimum 1 year Home Health experience required
