Currently covering 13 counties in Illinois:
Cook, DuPage, Kane, Kendall, Will, McHenry, Lake, DeKalb, Boone, Grundy, LaSalle, Lee & Ogle.

Provides Speech-Language Pathology services to patients under the direction and supervision of a licensed Speech-Language Pathologist, in accordance with the plan of care and in line with the philosophy, objectives, and policies of the agency.

• Implements the treatment plan established by the supervising Speech-Language Pathologist.

• Provides speech therapy treatments and interventions as outlined in the plan of care.

• Assists patients with speech, language, cognitive-communication, and swallowing exercises as directed.

• Conducts therapy activities focused on articulation, fluency, voice, and language skills.

• Documents patient progress, treatment provided, and any changes in patient condition.

• Reports observations and patient responses to the supervising Speech-Language Pathologist.

• Communicates effectively with patients, caregivers, families, and agency staff.

• Instructs patients and/or family members in home exercise programs as directed by the SLP.

• Coordinates care with agency DON and other staff as appropriate.

• Observes, records, and reports any changes in patient condition to the supervising SLP and agency.

• Participates in supervisory visits with the Speech-Language Pathologist as required.

• Assures that all tasks are performed in a timely manner.

• Provides quality and appropriate care and reports issues or concerns to the DON/Administrator.

• Must be a licensed Speech-Language Pathology Assistant (SLPA) by the State of Illinois.

• Must have at least one year of Home Health Care clinical experience.

• Car with required proof of insurance coverage.

• Maintains annual TB test, CPR certification, and other requirements.

• Able to read and interpret instructions related to patient care.

• Able to effectively perform and prioritize multiple functions or tasks as needed.

• Must be able to travel to patient's place of residence.

Pay: $65,000 - $75,000 per year
Schedule: Monday to Friday;
Weekends as needed

Experience: Minimum 1 year Home Health experience required
