Job Summary

Job Summary: The Coordinator-RN Heart Failure (HF) Program is an experienced nurse with extensive knowledge in the field of cardiology with a focus in adult heart failure disease. Serves in a patient-centered care model of practice in a Magnet Recognized large academic medical center with Level One Trauma Designation, ST-Elevation Myocardial Infarction (STEMI) Receiving Center Designation, Chest Pain Center Accreditation, Advanced Certification in Ventricular Assist Device (VAD), Adult ECMO Program and Comprehensive Stroke Center (CSC) Accreditation.

Works collaboratively with the Director, Administrative Director, Medical Director and other key individuals to acquire and maintain HF accreditation through the American College of Cardiology (ACC). Maintains electronic HF database(s) as required for program accreditation and tracking performance metrics. Benchmarks data with local, regional, and national databases. Utilizes discretion and independent judgement to apply specialized HF case management and care coordination methodologies and practices for a seamless multidisciplinary approach to care.

Ensures effective utilization of resources and provides cutting-edge quality care. Collaborates with physicians, nurse practitioners and payers to follow patients to ensure continuity of care. Serves as a clinical HF resource and a liaison among physicians, staff, patients, families and payers to ensure effective utilization of resources and optimal care coordination. Supports clinical and translational research activities and participates in Institutional Review Board (IRB) approved research that improves outcomes and quality of life for patients with HF.

Facilitates the integration of research and evidenced-based practice to optimize patient outcomes. Participates in local, national and worldwide activities and initiatives to develop networks and alliances that promote LLUMC's HF program while augmenting visibility and enhancing service line and program image. Contributes to planning seminars, conferences and symposiums for educational events related to cardiovascular programs. Demonstrates a commitment to excellence and the exceeding of customer expectations by modeling behavior in the context of institutional values, empowerment, alignment, creativity and education.

Performs other duties as needed.

Education and Experience
  • Bachelor's Degree in Nursing required.
  • Master's Degree in Nursing preferred.
  • Minimum of two years' experience caring for cardiology patients with cardiac diseases preferred.
  • Case Management experience preferred.
Knowledge and Skills
  • Substantial knowledge of heart failure disease and cardiomyopathy.
  • Knowledge of state and federal healthcare regulations and accreditation, finance, performance improvement, quality, patient safety, evidenced-based clinical practice standards, medical staff structure, legal aspects of care, and nursing scope of practice required.
  • Ability to apply theoretical concepts to clinical practice and continually expand knowledge base and clinical skills with a focus in area of specialty and population served.
  • Be assertive and consistent in following and enforcing policies; recall information with accuracy.
  • Use computer and software programs necessary to the position (e.g., Word, Excel, Power Point, Access); operate/troubleshoot basic office equipment required for the position.
  • Able to relate and communicate positively, effectively, and professionally with others; provide leadership; be assertive and consistent in enforcing policies ensuring compliance with regulatory standards, rules and laws; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction.
  • Able to communicate effectively in English in person, in writing, and on the telephone; think critically; work independently; perform basic math and statistical functions; manage multiple assignments; compose written material; work well under pressure; problem solve; organize and prioritize workload; recall information with accuracy; pay close attention to detail.
  • Able to distinguish colors as necessary; hear sufficiently for general conversation in person and on the telephone, and identify and distinguish various sounds associated with the workplace; see adequately to read computer screens, and written documents necessary to the position.
Licenses and Certifications
  • Active California Registered Nurse (RN) license required.
  • Basic Life Support (BLS) certification issued by the American Heart Association required.
  • Certified Case Manager (CCM) preferred.
