Job Description
The Patient Flow Facilitator (PFF)-RN is a highly skilled licensed registered nurse that provides care to patients admitted to Loma Linda University Medical Center (LLUMC). The PFF functions out of a home department and floats to all areas of the hospital with a primary focus on patient flow. Responsibilities include maintaining the specified level of care while transporting patients with admission orders from the emergency department to the unit, transporting patients within LLUMC units, and assisting with expediting admissions, transfers, and discharges.
Depending on business needs, the PFF may serve in the hospitality/discharge lounge or accept a full or partial patient assignment on a unit. The PFF practices in a patient-centered care model, performs the nursing process independently, and serves as a model of clinical practice, communication, and documentation. The role supports improvements in practice to enhance patient experience and outcomes, with a focus in the pediatric population.
The PFF uses interpersonal skills to maximize customer service, safe patient care, and professionalism, while contributing to a caring and cooperative work environment across a diverse workforce and patient population. Performs other duties as needed.
