Job Summary: The Supervisor—Case Management at Loma Linda University Medical Center is responsible for leading case management and social work staff, ensuring patient transition, utilization review, and discharge planning while maintaining compliance with state and federal regulations and organizational policies.
- Lead and supervise day‑to‑day activities of the in‑house CM and SW staff.
- Coordinate care management, utilization review, and discharge planning to achieve clinical outcomes and patient safety.
- Maintain collaborative relationships with physicians, nursing, social work, and other multidisciplinary teams.
- Serve as a resource to remove barriers to patient placement and discharge.
- Represent the patient experience, influencing satisfaction ratings, revenue, and cost reduction.
- Assist the Director of Case Management with employee supervision and leadership development.
- Perform additional duties as needed.
Education and Experience: Bachelor’s Degree in Nursing required;
Master’s degree preferred. Minimum two years of case management experience in an acute‑care hospital. Case Management certification preferred. Utilization Management knowledge required.
Knowledge and
Skills:
Substantial knowledge of case and utilization management, social work, healthcare regulations, finance, quality improvement, and evidence‑based standards. Knowledge of adult learning principles and problem‑solving methodologies. Proficient in Microsoft Office, electronic medical record, and reporting tools. Strong communication skills in English;
Spanish language preferred. Ability to work independently, manage multiple assignments, and enforce policies.
Licensures and
Certifications:
Active California RN license. Basic Life Support certification.
