The Disabled Children's service supports children who have a lifelong, permanent and substantial disability. The service is made up of five teams:
Enablement Team, 0-11 Team, 11+ Team, Eligibility Team and Occupational Therapy Team. The 11+ team specialises in delivering support to children and young people through transitional milestones. Supporting children from the age of 11 years through to 18 years.
The primary aim of the 11+ team is to provide care and support to children and young people with complex physical and learning disabilities and/or life limiting conditions as they move from children's to adult services. They focus on promoting independence, managing complex needs, and preparing for adulthood, conducting assessments to create tailored support plans.
As part of their role in supporting young people through transition, the team work closely with Adult Services. Preparing for transition, in some cases, can involve the undertaking of mental capacity assessments and relevant administration to support applications to the Court of Protection.
