Description
Powys County Council is seeking a compassionate and experienced Social Worker to join our Older Persons Team (South). This is an exciting opportunity to work with individuals aged 65 and over, along with their carers, using a collaborative, strengths-based, and outcome-focused approach. The role covers Hay and Talgarth, Brecon, Crickhowell, and Ystradgynlais localities, supporting older adults to maintain independence, enhance well-being, and access the right care and support at the right time.
Responsibilities- Undertake assessments and reviews to identify individual needs, ensuring support is tailored to promote independence and well-being.
- Apply the Social Services and Well‑being (Wales) Act 2014 in practice, ensuring service users receive appropriate care and support.
- Work with individuals and their families to find sustainable solutions to challenges, both in the short and long term.
- Apply the principles of the Mental Capacity Act 2005, ensuring decision‑making is in the best interest of service users.
- Support individuals who may be at risk of abuse or neglect, taking appropriate safeguarding actions.
- Work in partnership with health colleagues, third‑sector organisations, and other agencies to provide a coordinated and proportionate response.
- Promote strengths‑based approaches to empower individuals and their carers to live fulfilling and independent lives.
- Qualifications:
Degree in Social Work and registration with Social Care Wales. - Experience:
Working with older adults and their carers in a social work setting. - Knowledge: A strong understanding of the Social Services and Well‑being (Wales) Act 2014, the Mental Capacity Act 2005, and safeguarding legislation.
- Skills:
Excellent assessment, problem‑solving, and communication skills, with the ability to engage effectively with service users and multi‑agency partners. - Other Requirements:
Commitment to person‑centred, strengths‑based, and outcome‑focused practice.
