• Provide nursing care and support to patients in their homes across Wayne, Oakland, and Macomb counties.
• Administer medications and treatments as prescribed by physicians.
• Monitor patient health and report any significant changes to the healthcare team.
• Maintain accurate and detailed patient records.
• Collaborate with other healthcare professionals to coordinate patient care.
• Educate patients and their families on health management, disease prevention, and post-treatment care.

• Current and valid LPN license.
• Experience in a home care or clinical nursing environment preferred.
• Strong communication and interpersonal skills.
• Ability to travel within Wayne, Oakland, and Macomb counties.
• Reliable and compassionate with a strong commitment to patient care.
