University Home Care Inc is seeking a compassionate and dedicated Hospice Registered Nurse to provide exceptional hospice care to patients and their families across Wayne, Oakland, and Macomb counties. This role involves delivering personalized nursing care in patients’ homes, ensuring comfort and dignity during end-of-life care. The Hospice Registered Nurse will collaborate with an interdisciplinary team to develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients.
Responsibilities- Provide comprehensive nursing assessments and develop individualized care plans for hospice patients.
- Administer medications and treatments as prescribed, adhering to hospice care protocols.
- Offer education, emotional support, and counseling to patients and their families.
- Coordinate patient care with physicians, social workers, chaplains, and other team members.
- Monitor and document patient progress, symptoms, and any changes in condition.
- Respond promptly to patient needs and emergencies, providing expert nursing care.
- Maintain compliance with all regulatory requirements and company policies.
- Valid Registered Nurse (RN) license in the state of Michigan.
- Previous experience in hospice or palliative nursing preferred.
- Excellent interpersonal and communication skills to support patients and families effectively.
- Ability to work independently and as part of a multidisciplinary team.
- Reliable transportation for travel within Wayne, Oakland, and Macomb counties.
- Strong organizational and documentation skills.
- Competitive compensation ranging from $65.00 to $150.00 per visit, based on experience and visit complexity.
