Hospice Admission Nurse (RN)
Basic Job Function Summary
The Hospice Admission Nurse (RN) provides compassionate, patient-centered care by utilizing critical thinking skills to perform comprehensive assessments, develop individualized plans of care, and deliver high-quality hospice services. This role ensures comfort and dignity for patients and their families in alignment with the interdisciplinary care plan and regulatory standards. The Admission RN also coordinates and communicates effectively to establish the initial hospice plan of care.
Supervision Exercised
Provides functional supervision of Licensed Practical Nurses (LPNs) and Hospice Aides.
Minimum Qualifications
* Graduate of an accredited nursing program.
* Current Registered Nurse (RN) license in the state of practice.
* California:
Minimum one year of professional nursing experience required by regulation.
* At least one (1) year of acute care nursing experience.
* Home health or hospice experience preferred.
* Strong communication and interpersonal skills, with sensitivity to grief and loss.
* Valid driver's license and reliable transportation.
* Commitment to organizational mission, ethics, and compliance standards.
Essential Functions
* Conducts comprehensive assessments of patient and family needs-physical, emotional, social, and spiritual-and collaborates with the interdisciplinary team (IDG) to develop and implement the plan of care.
* Maintains timely and accurate documentation of patient status, interventions, and goals in compliance with agency protocols and regulatory requirements.
* Communicates effectively with physicians, IDG members, hospice leadership, and other care providers to ensure coordinated, optimal care.
* Provides education and support to patients and caregivers based on individualized learning needs.
* Participates in on-call and admissions rotation as assigned.
* Coordinates admission activities, including informational visits, consent, initial plan of care, and discharge planning.
* Engages in quality improvement initiatives, ongoing education, and committee participation as needed.
* Adheres to agency and state policies governing hospice care delivery.
* Performs other duties within the scope of licensure as assigned.
