Overview
Wayne County (limited Oakland & Washtenaw border areas). M & M Home Care is seeking an experienced Physical Therapist (PT) to join our team in a PRN role serving clients primarily in Wayne County, with occasional coverage in adjacent border areas of Oakland and Washtenaw counties. This role is ideal for a PT who values clinical autonomy, flexible scheduling, and meaningful 1:1 care — while still having strong support and mentorship readily available.

Our clients are predominantly survivors of motor vehicle accidents with complex injuries requiring thoughtful, individualized rehabilitation. - PRN position
- $80/hour
- Minimum of ~4 visits per week to start, with room for growth
- You set your own schedule
- Paid drive time and travel reimbursement

You will work with clients experiencing:
- Traumatic brain injuries (TBI)
- Spinal cord injuries (SCI)
- Complex orthopedic injuries and fractures
- Amputations
- Chronic pain and post-traumatic arthritis
Care focuses on rehabilitation progression, long-term functional improvement, and high-quality chronic care when appropriate. Some awareness of wheelchair seating is helpful, though formal wheelchair evaluations are supported by a specialist whom we have on staff.
Support & OnboardingWhile this role offers significant independence, you are never on your own. Support includes:
- Structured onboarding with the Director of Therapy
- Field shadowing with an experienced PT
- Ongoing mentorship from PT leadership
- Regular check-ins with the Director and Assistant Therapy Director
- A collaborative interdisciplinary team (OT, SLP, massage therapists, licensed professional counselors, nursing, case managers, physicians)
- Provide direct 1:1 patient care in the home setting
- Perform comprehensive evaluations and develop individualized plans of care
- Collaborate with the interdisciplinary care team
- Adjust care plans as needed to support optimal outcomes
- Maintain timely, accurate clinical documentation
- Practice in accordance with the Michigan Physical Therapy Practice Act
- Competitive hourly pay ($80/hr)

- Graduate of an accredited Physical Therapy program
- Current Michigan PT license in good standing
- Prefer 1 year of clinical experience
- Experience with TBI and/or SCI strongly preferred
- CBIS certification and/or OMPT training a plus, not required
- Strong time management and ability to work independently in the field
