The Case Manager Registered Nurse will coordinate discharge planning to reduce LOS and improve overall patient care while contributing to financial stability of the organization.
Qualifications- Baccalaureate degree in Nursing from an accredited school or university, or equivalent RN degree with commensurate clinical experience
- Minimum of 2-3 years' experience in a hospital setting
- Minimum of one year experience in Case Management/Utilization Management or Home Care
- Successful completion of all Orientation Programs
- Current Registered Nurse License in the state of New Jersey
- Basic Life Support (BLS) from the American Heart Association
- Full-time
- Day shift
- 37.5 hrs weekly
- 8AM-4PM, Mon-Fri
- Weekend commitment as needed
- Holidays as needed
The Case Manager coordinates the healthcare needs of patients in the hospital by collaborating with members of the interdisciplinary Patient Care Team and the Primary Care Physician on the in-hospital and post-discharge plan of care. They coordinate the care and services of the Cooperman Barnabas Medical Center patient population across the continuum of illness, assume responsibility for utilization management and monitoring processes, and conduct admission and stay reviews per program contracts.
They also identify trends in resource use and patient care, provide recommendations in compliance with policy, and ensure continuity of care for patients with post-hospital needs.
