RequiredCertifications and Licenses/Experience
- Baccalaureate degree or higher
- Graduate of an accredited school of nursing
- Currently licensed to practice professional nursing in the State of New Jersey
- BLS Certification Required
- Previous oncology experience and OCN preferred
- Full time - Days
- Monday - Friday
- Contributes to the care of any hospitalized cancer patients by engaging patients in the transitional care management process and by collaborating with the outpatient clinicians. This person meets with patients upon admission to the inpatient unit, participates in multidisciplinary daily rounds, ensures the coordination and confirmation of appointments and follow‑up requirements for discharge, including but not limited to:
- Identify patients at an increased risk for discharge barriers or readmission
- Clinic appointments
- Treatment appointments
- Diagnostic tests
- Consultation appointments
- Radiation/Oncology appointment
- Referring community provider appointments
- Confirmation of home needs
- Review discharge checklist and instructions with patients and caregivers
The Oncology Care Transition Coordinator is a registered professional nurse who under the supervision of the Oncology Care Transitions Manager contributes to the care of any hospitalized cancer patients by engaging patients in the transitional care management process and by collaborating with the inpatient and ambulatory clinicians. This person meets with patients upon admission to the inpatient unit, participates in multidisciplinary daily rounds, ensures the coordination and confirmation of appointments and follow‑up requirements.
