The experienced Stroke Program Coordinator is responsible for coordinating initiatives to achieve, maintain, and continuously improve the hospital’s Joint Commission–certified Comprehensive Stroke Program
. The role focuses on program coordination, regulatory compliance, performance improvement, data analysis, and education
, ensuring adherence to TJC Comprehensive Stroke Center standards
, New Jersey stroke regulations
, and evidence-based stroke care guidelines.
The Stroke Coordinator works collaboratively with the Lead Stroke APP, Stroke Medical Director, interdisciplinary teams, and hospital leadership to ensure sustained survey readiness and optimal patient outcomes.
Responsibilities- Stroke Program Coordination & Oversight:
Coordinate initiatives to achieve and maintain Comprehensive Stroke Center certification. - Support implementation, maintenance, and revision of stroke policies, procedures, and protocols.
- Maintain confidential and accurate minutes for the Stroke Committee and related meetings.
- Track and ensure follow-up of all outstanding agenda items until resolution and closure.
- Escalate identified issues to the appropriate individual or department in a timely manner.
- Oversee stroke data abstraction, submission, and validation (e.g., Get With The Guidelines–Stroke).
- Maintain passing averages and inter-rater reliability for stroke audits and abstractions.
- Prepare performance improvement (PI) reports, power points, dashboards, and regulatory documentation.
- Review stroke data on a regular basis to determine compliance, identify performance gaps, and develop and monitor corrective action plans.
- Ensure compliance with TJC Comprehensive Stroke standards and New Jersey stroke measures.
- Review selected stroke-related mortalities to ensure adherence to standards of care.
- Provide follow-up and documentation for all PI initiatives until closure.
- Monitor adherence to evidence-based stroke guidelines for ischemic stroke, ICH, SAH, TIA and complex neurovascular cases.
- Support survey readiness activities including tracers, mock surveys, and regulatory audits.
- Collaborate with clinical teams to address opportunities related to patient safety, quality, and outcomes.
- Ensure documentation reflects compliance with regulatory, accreditation, and organizational standards.
Preferred:
- Performance improvement experience preferred
- Registered Nurse (RN) with an active New Jersey license and clinical stroke experience
- Speech-Language Pathologist (SLP) with an active New Jersey license and stroke or neuro experience
- Occupational Therapist (OT) with an active New Jersey license and stroke or neuro rehabilitation experience
- Physical Therapist (PT) with an active New Jersey license and stroke or neuro rehabilitation experience
- Rehab specialists (SLP, OT, PT) must maintain all required certifications, credentials, and continuing education necessary for licensure and practice.
- Bachelor’s degree or higher in a healthcare-related field, or equivalent clinical experience
- Experience in stroke program coordination, neuroscience, quality improvement, or acute stroke care
- Familiarity with Joint Commission Comprehensive Stroke Center standards
- Knowledge of New Jersey stroke measures and reporting requirements
