Overview
Ultimate Staffing Services is actively seeking a passionate and dedicated Quality & Population Health Specialist to join a mission-driven team in California. The Specialist will lead quality improvement and population health activities, driving performance improvements and advancing health outcomes across all service lines.
Responsibilities- Lead the development, implementation, and annual update of the organization's Quality Improvement and Population Health Plan, aligning with HRSA, FTCA, PCMH, and LCH strategic goals.
- Act as the primary subject matter expert on quality and population health, guiding leaders, managers, and staff in achieving quality targets.
- Collaborate with the COO, CMO, and operational leaders to integrate quality objectives into daily operations.
- Chair or co-chair the Quality Improvement Committee, managing agenda planning, data reporting, documentation, and follow-up on action items.
- Oversee data integrity for quality-related reports and dashboards, collaborating with IT/Data Analytics teams.
- Manage quality performance reporting cycles, including internal scorecards, board reports, and UDS submissions.
- Maintain readiness for HRSA Operational Site Visits and other audits, coordinating all quality-related components.
- Design and manage population health initiatives aimed at improving chronic disease outcomes and reducing health disparities.
- Coordinate and manage the provider peer quality review process, ensuring results support clinical quality improvement.
- Collaborate with external stakeholders, representing LCH in quality-related matters and aligning goals with health plan quality measures.
- Provide training and technical assistance to site managers, providers, and care teams on quality metrics and best practices.
- Bachelor's degree in Nursing, Public Health, Health Administration, or related field (or equivalent combination of education and experience).
- Minimum of 2 years of experience in healthcare quality improvement, population health, or clinical operations in an FQHC or similar setting.
- Basic Life Support (BLS) Certification from the AHA is preferred.
- Valid driver's license.
The position operates during 1st shift hours, Monday through Friday.
