Overview
We are looking for an experienced and motivated Practice Nurse to join our friendly primary care team on a 12‑month maternity cover contract. The role is suitable for a Band 6 or Band 7 nurse, depending on experience and scope of practice. The post holder will provide high‑quality, evidence‑based nursing care to the practice population, working autonomously within agreed protocols and as part of a multidisciplinary team.
Key Responsibilities- Provide high‑quality nursing care to patients across the practice.
- Undertake chronic disease management clinics (e.g. asthma, COPD, diabetes, hypertension).
- Deliver cervical cytology in line with national screening programmes.
- Administer childhood and adult immunisations and vaccinations.
- Support QOF and long‑term condition management.
- Maintain accurate and timely clinical records using the practice clinical system.
- Adhere to infection prevention and control policies.
- Contribute to clinical audits, service development and quality improvement initiatives.
- Participate in policy development and review.
- Work in accordance with the NMC Code of Professional Conduct; maintain registration, participate in revalidation, appraisal, training and clinical supervision.
- Contribute to safeguarding of children and vulnerable adults.
- Work in accordance with the NMC Code of Professional Conduct.
- Maintain NMC registration and participate in revalidation.
- Take responsibility for maintaining and developing clinical competencies.
- Participate in appraisal, training and clinical supervision.
- Contribute to safeguarding of children and vulnerable adults.
- Registered Nurse (Adult) with current NMC registration.
- Experience working as a Practice Nurse or within primary care.
- Competence in chronic disease management.
- Competence in immunisations and cervical cytology.
- Ability to work independently and as part of a multidisciplinary team.
- Good communication, organisational and IT skills.
- Experience using GP clinical systems.
- Willingness to undertake further training.
- Teaching or mentoring experience (desirable).
- Hours:
30–37.5 hours per week (negotiable). - Contract:
Fixed‑term 12 months (maternity cover). - Location:
Woolton House Medical Centre. - Salary:
Band 6 or Band 7.
