Practice Nurse

Overview

We are looking for an experienced and motivated Practice Nurse to join our friendly primary care team on a 12‑month maternity cover contract. The role is suitable for a Band 6 or Band 7 nurse, depending on experience and scope of practice. The post holder will provide high‑quality, evidence‑based nursing care to the practice population, working autonomously within agreed protocols and as part of a multidisciplinary team.

• Provide high‑quality nursing care to patients across the practice.
• Undertake chronic disease management clinics (e.g. asthma, COPD, diabetes, hypertension).
• Deliver cervical cytology in line with national screening programmes.
• Administer childhood and adult immunisations and vaccinations.
• Support QOF and long‑term condition management.
• Maintain accurate and timely clinical records using the practice clinical system.
• Adhere to infection prevention and control policies.
• Contribute to clinical audits, service development and quality improvement initiatives.
• Participate in policy development and review.
• Work in accordance with the NMC Code of Professional Conduct; maintain registration, participate in revalidation, appraisal, training and clinical supervision.
• Contribute to safeguarding of children and vulnerable adults.

• Work in accordance with the NMC Code of Professional Conduct.

• Take responsibility for maintaining and developing clinical competencies.
• Participate in appraisal, training and clinical supervision.
• Contribute to safeguarding of children and vulnerable adults.

• Registered Nurse (Adult) with current NMC registration.
• Experience working as a Practice Nurse or within primary care.
• Competence in chronic disease management.
• Competence in immunisations and cervical cytology.
• Ability to work independently and as part of a multidisciplinary team.
• Good communication, organisational and IT skills.
• Experience using GP clinical systems.
• Willingness to undertake further training.
• Teaching or mentoring experience (desirable).

• Hours:
  
  30–37.5 hours per week (negotiable).
• Contract:
  
  Fixed‑term 12 months (maternity cover).
