Job Opportunity

An exciting opportunity to join our Integrated Heart Failure Service at Liverpool University Hospital Foundation NHS Trust based at the Aintree hospital.

We are seeking a highly motivated individual with excellent interpersonal and communication skills.

• Develop a cohesive management programme for patients with heart failure in line with local and national guidelines, working across acute, community and virtual ward settings.
• Operate the service 7 days per week, requiring flexibility and working within an evolving proactive, dynamic multidisciplinary Heart failure Team.
• Prevent unnecessary admissions to hospital and improve Heart failure management and control, optimising evidence‑based treatments.
• Provide expertise and advice in the treatment of highly complex conditions, undertaking treatments as required.
• Serve as a source of training and mentorship to those within the immediate team/area and, where necessary, more widely.
• Ensure delivery of the quality strategy within the designated area, incorporating establishment of systems and processes.

• NMC registered nurse.
• Physical health assessment skills.
• Non‑medical prescriber.
• Strong clinical reasoning and prioritisation skills.
• Ability to work effectively both independently and as part of a team.

Annex 21 position will be considered. Liverpool University Hospitals NHS Foundation Trust comprises Aintree University Hospital, Broadgreen Hospital & Royal Liverpool University Hospital. We are part of NHS University Hospitals of Liverpool Group, formed on 1 Nov 2024.
