Job Summary
We are seeking an experienced and motivated Band 6/7 Practice Nurse to join our friendly and supportive GP practice team at Woolton House Medical Centre. The post holder will deliver high‑quality, evidence‑based nursing care to our practice population, working autonomously within agreed protocols and as part of the wider multidisciplinary team.
Key Responsibilities- Provide high‑quality nursing care to patients across the practice population
- Undertake chronic disease management clinics (e.g. asthma, COPD, diabetes, hypertension)
- Deliver cervical cytology in line with national screening programmes
- Administer childhood and adult immunisations and vaccinations
- Carry out health promotion and disease prevention activities
- Manage long‑term conditions in accordance with evidence‑based protocols and QOF requirements
- Support infection prevention and control standards within the practice
- Maintain accurate and timely clinical records using the practice clinical system
- Contribute to clinical audits, service development and quality improvement initiatives
- Participate in policy development and review
- Work in accordance with the NMC Code of Professional Conduct
- Maintain NMC registration and participate in revalidation
- Take responsibility for maintaining and developing clinical competencies
- Participate in appraisal, training and clinical supervision
- Contribute to safeguarding of children and vulnerable adults
- Essential
- Registered Nurse (Adult) with current NMC registration
- Experience working as a Practice Nurse or within primary care
- Competence in chronic disease management
- Competence in immunisations and cervical cytology
- Ability to work independently and as part of a multidisciplinary team
- Good communication, organisational and IT skills
- Desirable
- Experience using GP clinical systems
- Willingness to undertake further training
- Teaching or mentoring experience
- Friendly, supportive and inclusive practice environment
- Strong teamwork and peer support

Fixed‑term 12 months (maternity cover). 30–37.5 hours per week (negotiable). Salary:
Band 6/7.
