The Senior Practice Nurse is an experienced autonomous practitioner and non medical prescriber with strong expertise in General Practice and a thorough understanding of the Quality Outcomes Framework and wider quality targets. They deliver advanced clinical care for complex chronic conditions, particularly for housebound patients, while working effectively within multi professional and multiagency settings. Operating at an advanced level of practice, they provide leadership through staff training, supervision, and support, and contribute to clinical audits, research, and service improvement.

The role also involves developing and updating local protocols, guidelines, and procedures to ensure high quality, evidence based care.

Main duties of the job

Undertake complex general practice nursing interventions across a wide range of clinical presentations, applying advanced practical skills and a critical understanding of the theories underpinning primary care.

Assess patients capacity for informed consent and support colleagues in developing this competency.

Apply highly developed specialist knowledge of longterm conditions, health behaviours, and evidence based interventions, including the use of technology to support patients at home.

Independently manage clinical care episodes from initial presentation through to completion, including referral, admission, discharge, or ongoing homebased care.

Use advanced therapeutic knowledge to prescribe and evaluate pharmacological and non-pharmacological treatments, assessing patient concordance and adjusting care plans accordingly.

Challenge practice, systems, and policies constructively and objectively to support continuous improvement.

Contribute to the development and review of local policies, pathways, and procedures.

Design, deliver, and evaluate training and education for individuals and groups, working collaboratively across disciplines to support interprofessional learning.

Demonstrate originality and innovation in service development, ensuring safe implementation of new guidelines, policies, and models of care.

Job responsibilities

Able to undertake complex general practice nursing interventions for a wide range of general practice issues andteach and develop other staff to enhance their practice to manage futuresituations. This will entail being able to demonstrate not only practicalknowledge but also a critical understanding of the range of theories and principles that underpin the general practice nursing approach.

Highly developed specialist knowledge and understanding of LTCs, health behaviour and interventions to improve health outcomes, including the use of technologies tosupport patients at home.

Ableto manage clinical care at the highest level independently, from beginning toend, for example from when an individual presents through to the end of theepisode, which may include admission, referral or discharge or care at home.

Advanced knowledge and skills in therapeutics to prescribe effective pharmacological andnon- pharmacological approaches for the management of LTCs and assess patientconcordance.

Is able torespectfully challenge practice, systems and policies in an objective and constructive manner.

Participate inopportunities to influence local policies, pathways and procedures.

Is able todevelop, deliver and evaluate training and education packages, for individuals and groups, across a broad range of general practice nursing needs and in collaboration with other disciplines and agencies to facilitateinter-professional/agency learning.
