The Role
We are seeking a motivated and compassionate Practice Nurse to join our clinical team. The successful candidate will play a key role in providing high standards of nursing care, health promotion, and chronic disease management.
Main duties of the jobKey Responsibilities
- Chronic disease management (e.g. asthma, COPD, diabetes, hypertension, coronary heart disease)
- Immunisations and vaccinations in accordance with national and local programmes
- Cervical screening and women’s health services (where trained)
- Wound care and dressings
- Health promotion and lifestyle advice
- Administer injections, venepuncture, and infection control
- Support GMS contract, QI projects and enhanced services delivery
- Work collaboratively with GPs, healthcare assistants, pharmacists, and the wider primary care team
- Maintain accurate and up-to-date clinical records using electronic patient record systems
- Ensure compliance with safeguarding policies for children and vulnerable adults
- Participate in clinical audit, quality improvement, and service development
- Maintain professional development, NMC revalidation, and mandatory training
Waunfawr Surgery is a friendly, well-established GP practice serving a diverse patient population in a semi rural area. We are committed to delivering high-quality, patient-centred care and are supported by an experienced multidisciplinary team including GPs, nurses, healthcare assistants, and administrative staff.
The practice has:
5 GP partners, 2 ANPs, 1 APP, 4 PNs and 3 HCAs
Job responsibilitiesLocation – Meddygfa Waunfawr, Liverpool House, Waunfawr Reports to – GP Partners / Practice Manager
Person SpecificationRegistered General Nurse (RGN) with current NMC registration.
Excellent communication and organisational skills.
Ability to work independently and as part of a multidisciplinary team.
Experience working in primary care or general practice.
Training in chronic disease management, immunisations, or cervical screening.
Knowledge of NHS Wales systems and primary care pathways.
