Patient Flow Coordinator
Closing date: 1 February 2026. Shift: 08:00-16:00, Mon‑Fri.
Role OverviewSupport patient flow and discharge management within ward department, ensuring timely coordinated discharges and effective communication among multidisciplinary teams and external stakeholders.
Main responsibilities- Optimise patient flow to improve bed availability and reduce delays.
- Manage safe and timely discharges across Pathways 1, 2, and 3.
- Provide essential MDT support by coordinating discussions and decision‑making.
- Facilitate Best Interest Meetings (BIM) to ensure patient‑centred discharge planning.
- Maintain momentum of discharge planning throughout the entire process and support other MDT members.
- Develop and maintain productive integrated working relationships with social service and community healthcare colleagues.
- Co‑ordinate timely assessment and safe transfer or discharge of people from the ward department.
- Demonstrated willingness & capability to undertake NVQ level 3.
- Demonstrable experience in hospital setting working at NVQ level 2.
- Reputation for high standard of patient care.
