Community Clinical Support Worker

Job Summary

We are inviting applications for a highly motivated and dynamic Health Care Assistant to join North Tees & Hartlepool Community Nursing Service. This service will provide care to patients within a range of settings including patients’ own homes and residential care homes. The post holder will have a high level of knowledge in community care with specific interest in areas such as palliative care, long-term conditions, catheterisation and wound care.

You will be required to work autonomously to provide a whole person approach to care and management for patients within this setting.

Main duties of the job

The post involves liaising with a variety of health and social care providers and other agencies to ensure the provision of high quality care management to this group of patients and their carers.

Job responsibilities

We require a Health Care Assistant with NVQ level 3 in health and social care who has excellent communication skills and is confident working across professional boundaries. You will need to be flexible, adaptable and a good team player with the ability to work independently within a community setting. You will be prepared to work all shift patterns within the locality including early and late shifts, weekends and bank holidays.

There may also be the need to provide cover for the out of hours District Nursing team.

Qualification

Essential

• NVQ 3 or alternative equivalent qualifications / experience.
• Good standard of literacy / numeracy.
• Able to use IT systems such as email and internet to find information and the patient’s electronic record.

Experience

• Demonstrate sound basic knowledge and understanding of a broad range of medical conditions particularly those affecting older people.
• Evidence of ability to work with a multi disciplinary team.
• Previous recent experience in a clinical setting.
