Responsibilities- Record patient fluid intake and output using fluid balance charts.
- Record dietary intake when indicated in the patient’s plan of care.
- Prepare and serve patients’ meals, drinks and snacks; assist/ feed patients as necessary.
- Enable patients to maintain or improve mobility by using exercise and mobility appliances.
- Assist with personal hygiene, grooming, toileting and incontinence management.
- Help prevent pressure sore development in accordance with the Registered Nurse and the patient’s plan of care.
- Carry out movement and handling within safe practice guidelines and the patient’s manual handling plan.
- Undertake dressing of non‑complex wounds as per identified competencies and the plan of care.
- Prepare and maintain environments for clinical treatments and investigations.
- Collect blood products from the laboratory as requested.
- Assist registered nurses in performing last offices.
- Observe and record vital signs: blood pressure, pulse, temperature, respiration rate, oxygen saturation, peak flow, and blood glucose monitoring (venepuncture and/or cannulation).
- Report any untoward events to registered nursing staff.
The frailty assessment team is a multidisciplinary group caring for patients aged 65+ with a clinical frailty score of 5‑7, from community, accident and emergency, and an ambulatory setting, as well as a 14‑bedded 72‑hour stay unit.
