Deliver proactive clinical pharmacy services to patients across the Trust. Work will be based mainly on the clinical wards working with the multidisciplinary team to review medication to ensure it's safe and effective.

Responsibilities
  • Take medication histories from patients on admission
  • Review patients' medication to ensure safe and effective use
  • Formulate individual patient pharmaceutical care plans
  • Perform therapeutic level monitoring for specific medicines
  • Educate patients about their medicinal therapy
  • Participate in consultant ward rounds
  • Facilitate implementation of local and national guidelines where appropriate
  • Challenge inappropriate prescribing
  • Deliver clinical and where appropriate final dispensing checks within the systems for dispensing & issuing medication from the pharmacy, in accordance with the prevailing safe systems of work
  • Provide professional and legal clinical supervision in the dispensary as allocated
  • Participate in, and supervise technical staff in, patient counselling to outpatient, inpatients and day‑case patients

Qualifications – Desirable
  • Teaching/training/mentoring skills
  • General level knowledge of medical and surgical medicines issues
  • Knowledge of the law relating to pharmacy and medicines
  • Appreciation of pharmacoeconomic principles
  • Previous hospital experience
  • Vocational masters degree in pharmacy
