A health and social care charity is looking for an Assessment and Support Planning Specialist in Liverpool. This full-time role (35 hr/week) requires a driver and focuses on completing person-centred assessments to enhance care quality. The ideal candidate will have a Level 3 qualification in Health & Social Care and at least 2 years of relevant experience. You will work closely with individuals to ensure their needs are prioritized in support planning.
