You will provide specialist care and support and deliver our vision to create a world where people control their lives in line with each individual's personalised plan.

Responsibilities
  • Support a young person with Autism who requires 24-hour support and may exhibit challenging behaviours, in a newly developed, state-of-the-art tech‑equipped home.
  • Provide all health-related and social support to people with diagnoses such as Learning Disability, Autism, and Mental Health.
  • Support people who present with behaviours that challenge using appropriate techniques and strategies.
  • Provide safe medication and pain management to the people we support in line with their medication care plan.
  • Manage medical equipment and supplies and monitor depletion, re‑ordering as necessary in a timely manner to ensure nothing runs low.
  • Attend all clinical mandatory and specialist training sessions, face to face and online, to remain competent to deliver care to the people we support.
  • Ensure all incidents which cause or have the potential to cause harm are reported and recorded in a timely manner, in line with AFG policies and procedures.
  • Support people to design, develop and own the support they receive, making an effective contribution to Personalised Planning.
  • Support people to make informed choices and increase their independence and to follow the principles of the Mental Capacity Act when someone has difficulty making a specific decision.
  • Support people to exercise their cultural beliefs and practices.
  • Enable people to form and maintain relationships of their choice.
  • Support people to maintain their home/living environment.
  • Promote and increase independence for the people we support.
  • Support people to access their community.
  • Enable the people we support to take positive risks in their lives.
  • Ensure the people we support are not abused, neglected or ill‑treated.
  • Stop any such poor treatment, make the situation safe and report any concerns to line management, On Call Manager or, if need be, through Whistleblowing procedures.
Training & Development
  • Participate fully in training and development in accordance with the Alternative Futures Group development programme and any external training as required.
  • Attend and actively participate in supervision sessions and performance reviews.
  • Attend and actively participate in team meetings.
  • Develop and share skills to aid in the provision and improvement of support.
Essential Criteria
  • Ability to support people in ways consistent with their rights, choices and responsibilities.
  • A positive attitude to people with complex physical, learning disabilities and / or mental health problems.
  • Ability to provide sympathetic, emotional and practical support to others.
  • Ability to support others to make informed choices within their daily lives.
  • Good verbal communication skills and ability to listen sensitively to others.
  • Written English skills, with an ability to contribute accurately to record keeping.
  • Good interpersonal skills.
Experience
  • Minimum of 2 years experience in a health & social care paid or unpaid role.
Personal Attributes
  • Willingness to attend all clinical mandatory and specialist training courses and events.
  • Ability and willingness to work flexible hours, covering 24 hours/7 days.

Drivers are preferred for this role, as initially some travel will be required to attend induction training courses, however there are good public transport links nearby and driving isn't an ongoing requirement of the role.

Sleep-in shifts are paid at £12.60 per hour.
